I do not have any injuries or conditions that prevent me from receiving services. I understand the importance of informing my service provider of all medical conditions and medications that pose as contraindications to the service.*

I understand the risks associated with the requested beauty services.*

I further understand that the requested beauty services do not substitute medical examination or treatment and I should see a physician or other qualified health specialist for any ailment I am aware of. I understand the service provider will not diagnose any illness/diseases.*

I hereby grant to Elegant Beauty Collective, LLC the absolute and irrevocable right and unrestricted permission to publish, distribute, exhibit or otherwise use any photography and/or videography captured during services for use in educational and marketing purposes.

I have signed this electronic waiver and release freely, voluntarily, under no duress or threat of duress, without inducement, or guarantee being communicated to me. My electronic signature is proof of my intention to execute a complete and unconditional waiver and release of all liabilty to the full extent of the law.*
